NYSDOT SUNRISE HIGHWAY OPEN ELECTRIC CONDUIT

This project consists of installing approximately 300’ of 5”  PVC conduit in a trench with full restoration between manholes across Sunrise Highway on North Park Avenue. JOADEM Corporation was contracted by the Village of Rockville Center and approved by the NYSDOT to implement the Quality Control/Quality Assurance program throughout the entire project duration.

The scope of work entailed the installation of six 5″ PVC schedule 40 conduits on North Park Avenue from the southside of Sunrise Highway specifically manhole 1666 to the north side of Sunrise Highway manhole 1677. Additionally, four 5″ PVC schedule 40 conduits will go from manhole 1666 to manhole 588 and from 588 to 1677. Additionally, the repair of manhole 588 was required once the conduits have been installed.

JOADEM Corporation was hired to monitor the work of the Village of Rockville Center and Contractor to ensure that the work performed under the contract was done in accordance with the plans, the Standard Specifications, and all other requirements of the permit. This included; documenting and informing the NYSDOT of deficiencies in workmanship, material quality, Work Zone Traffic Control, Safety, etc.

Services performed under this contract include but not limited to:

• Perform construction inspection in accordance with the standard practices of NYSDOT.
• Obtain all necessary material samples and conduct all necessary material tests in accordance with NYSDOT’s materials methods.
• Utilize NYSDOT inspection form and maintain records in accordance with the current NYSDOT Manual of Uniform Record Keeping (MURK) on Highway Contracts.
• Review and inspect compliance with all aspects of the Work Zone Traffic Control provisions of the Plans, the Permit, MUTCD and NYSDOT Standard Specifications and notify NYSDOT of any noncompliance issues.
• Monitor the Contractor’s efforts to maintain traffic and protect the public from damage and inform the NYSDOT of any violations in the performance of the work on this project which failed to be immediately corrected.
• Prepare all drawings, sketches, and plans necessary for changes to meet actual field conditions. Provide three sets of Record (As-Built) Plans upon completion of the work.
